Diabetes Management Resources
AIM Diabetes Management Clinic
Run by residents and Angelica Santana, a diabetes educator, this is a 30 minute visit dedicated exclusively to assessing diabetes management including glycemic control, eye and foot screening, and diet/medication adherence. Medications can be titrated at these visits. For patients with poorly controlled or difficult-to-manage diabetes (A1c > 9%), or for patients who need close follow up after medication changes. If referring for a specific reason or intervention, please make this clear in your progress note. Refer via the AIM order set to "Diabetes Management and Diabetes Self-Management Education/Training."
Diabetes Self-Management Education (DSME)
Individual visits with Angelica Santana for self-management education that targets patient specific barriers (medication adherence, glucose monitoring, healthy eating, exercise, etc). For any patient with diabetes! Refer via “Diabetes self-management education/training (DSME/T)” order in AIM order set.
AIM Nutrition Referral
For any diabetic or obese patient interested in improving his diet for management of diabetes. Refer via the AIM Order Set, under "On Site Referrals."
Naomi Berrie Diabetes Center
For patients with Type 1 diabetes, as well as brittle or otherwise difficult to manage Type 2 diabetes. Please refer via the AIM Order Set to Naomi Berrie for diabetes management.
